WAIVERS AND FORMS
In order to ensure that we can all relax and have fun together, while staying safe and informed, the following waivers and permissions are required for all minors spending time at Roots without a parent or legal guardian.
This is a one-time process.
You can click the icons below to download the forms. These forms are fillable once downloaded, so there is no need to print them.
Once you complete the forms, save them to your computer so you can upload them below. Please fill out your information and upload your forms so we have them on file.
NOTE: If you prefer to print the forms and fill out a hard copy, you can upload photos or scans of the completed forms.
